We do this by embodying our WE CARE values.As a part time Medical Social Worker you will be an integral part of the care coordination team, working in collaboration with RN case management, the clinical team, and internal/external collaterals. As a member of our interdisciplinary team you will be responsible for performing psychosocial assessments, providing supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations.Who We Are: At Beverly Hospital, part of Beth Israel Lahey Health system, we proudly provide leading-edge patient centered care to our North Shore and Cape Ann communities.
