Patient Coordination: Ability to manage care for patients with complex medical and social needs. ? Interpersonal Skills: Strong communication skills, including motivational interviewing and behavioral change management. ? Medical Terminology: Proficient understanding of medical terminology and primary care practices. ? Bilingual Abilities: Spanish language skills are an advantage. ? Technical Proficiency: Experience with electronic health records (EHR) systems (e.g., EPIC, Athena, Cerner) and various software applications. ? Remote Collaboration: Facilitate communication between patients, medical staff, and the Case Management team to enhance patient care. ? Patient Engagement: Guide patients through their healthcare journey, answering questions, addressing concerns, and executing effective care plans. ? Documentation Management: Accurately document patient medical histories and related issues in the electronic health record system. ? Referral Assistance: Navigate and assist in the referral and prior authorization processes to ensure seamless patient care transitions. ? Professional Relationship Building: Develop and maintain strong relationships with medical practices to promote positive patient outcomes. ? Effective Communication: Use calls, texts, and emails to communicate with patients, especially high-risk individuals, ensuring appropriate escalation of medical issues. ? Occasional Travel: Visit primary care physician offices as necessary to enhance continuity of care. ? Security Compliance: Adhere to policies and procedures to protect patient information and uphold organizational standards. Qualifications ? Certification: Must possess either a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification. ? Experience: Proven experience in a similar role or medical setting, demonstrating proficiency in patient care management. ? Empathy and Skills: Must exhibit a strong aptitude for understanding patient needs and facilitating appropriate care solutions. Education Requirements ? Degree: High school diploma or equivalent is required; additional education in health sciences or related field is preferred. Education Requirements Credential Category ? Certification: Must hold a current certification as a Medical Assistant (CMA) or Registered Medical Assistant (RMA). Experience Requirements ?
