Schedule Details: This is a pool position requiring a minimum of 6 shifts per month; increased shifts may be required as needed for staff illness or PTO coverage. Shifts are Monday-Friday 8-4:30. Employee would also be included in department weekend and holiday rotation. This is NOT a fully remote position; the candidate will need to come into the office at Waukesha Memorial Hospital. What You Will Do: This position requires on-site work in Waukesha, WI. The Remote Patient Monitoring (RPM) RN will assess patient-generated data provided via digital technology to manage the care of both acute and chronically ill patients. Responsibilities include providing individualized patient assessments, education, medication management, goal setting, and care planning. The RN will collaborate with physicians and care team members to educate and support self-management of medical conditions in the home environment, using motivational communication techniques and positive reinforcement. The enrolled patients will submit vital signs and symptoms, which the RN will interpret to address patient needs and improve clinical outcomes and patient experience. The RN will draw on clinical expertise for health management goals and will remotely monitor all data, responding to alerts efficiently and escalating care when necessary. Early detection and intervention may reduce hospital visits and readmissions. The RN will build long-term relationships with patients through regular communication, becoming a trusted member of their care team. Additional Requirements:

  • Graduate from an accredited School of Nursing;

Bachelor’s degree preferred.

  • 1-2 years’ experience in hospital or ambulatory care, working with patients with chronic diseases.
  • Experience in remote patient monitoring is highly desirable. Familiarity with electronic medical records, care management systems, and Epic is preferred.
  • Strong technological aptitude with the ability to learn new software and tools quickly.
